Audience: Individuals or organizations that are interested in donating to Phoenix Residence, Inc. Specifically, people who would prefer not to utilize PRI’s online donations function. They are presented this form as an alternative on the PRI website.

Purpose: During my internship, PRI noticed that they had a very old PDF donations form buried  within their website. This form was black and white and created sometime in the 1990s. I redesigned the document completely and slimmed down the information. The form is provided to the donator on PRI’s webpage as a PDF. They can print the PDF, fill out the information, and submit it by snail-mail. PRI felt that there are still donators that may feel more comfortable mailing in their donations as opposed to using an online donations function.
